RETURNING AWARDEE APPLICATION
Overview

Thank you for your interest in maintaining your organization's designation as a CHLM Employer of Excellence. Using this abbreviated application, organizations that have been previous recipients of the Employer of Excellence award are able to apply to maintain their status as an awardee beyond the initial two-year time period.

New applicants and organizations that previously applied but were not selected to become awardees are not eligible to use this abbreviated application and must complete the CHLM Employer of Excellence New Awardee Application.

The application process is simple, requiring two steps:

  1. Provide employer contact details and brief demographic information.
  2. Describe the impact of receiving your previous Employer of Excellence Award.

The application may be saved after each step is completed. Upon exiting the platform, a link will be emailed to applicants so that they may return and complete the submission process.

Note: Applicants may return to add supporting documentation; however, all materials must be submitted by the application deadline of February 7, 2020 at 11:59 p.m. EST to be considered.
